Depression Diagnosis This is based on clinical features of core symptoms. A diagnosis of mild depression requires 2 of the 3 core symptoms for at least 2 weeks and 2 from the following list: o Reduced concentration o Reduced self esteem o Ideas of guilt and worthlessness o Feelings of hopelessness regarding the future o Thoughts of self harm o Disturbed sleeps o Increased or decreased appetite A diagnosis of moderate depression requires at least 2 of the 3 core symptoms for at least 2 weeks and 3 from the above list. A diagnosis of severe depression requires all of the core symptoms for at least 2 weeks and 5 from the above list; however, depression with psychosis is always classified as severe. Important Areas of Questioning (in patient history): DEAD SWAMP: Depressed mood most of the day How long have you felt like this? Do you feel worse at any time of the day? Energy loss or fatigue How are your energy levels?
