SLE This is a multi-systemic autotimmune disease in which antibodies are made against a variety of auto- antigens. Symptoms o Remitting relapsing course. o Non specific symptoms – malaise, fatigue, myalgia and fever. o Also lymphadenopathy and weight loss. o Specific features – skin and lung signs more common o Malar rash – fixed, erythematous, flat/raised. Naso-labial sparing. Occurs in 30%. o Discoid rash – erythematous raised patches with keratototic scales - affecting ears, cheeks, scalp, forehead and chest o Photosensitivity – exposure to sun causes flair o Oral ulcers o Non-erosive arthritis – tenderness swelling and effusion in 2 or more joints (seen in 90%) o Seroitis – causes pleuritis and pericarditis o Renal disorder – proteinuria or cellular casts. o CNS – seizure or psychosis Causes Poly-clonal B-cell secretion of pathogenic auto-antibodies results in formation of immune complexes which are deposited in sites such as kidneys.
