HUN4445 Estimating Protein and Fluid Needs, Biochemical and Hematological Assessment Objectives By the end of class, students will be able to: Estimate protein needs using the g/kg, nitrogen balance and kcal:N methods Estimate fluid needs Calculate nitrogen balance Describe the uses of the CBC and factors that influence Hemoglobin (Hgb) , Hematocrit (Hct) and Mean Corpuscular Volume (MCV) Interpret Hct, Hgb and MCV Assess the visceral protein status of patients using albumin and discuss factors that alter measures of visceral protein Estimating Protein Requirements 1. G/kg Body Weight Stress level Factor (g/kg) RDA 0.8 Repletion after illness or wt loss 1.2-1.5 Critically ill 1.5 Severe infection/burns ≥ 2.0 2. Nitrogen Balance Obtain urine urea nitrogen (UUN) Adjust for obligatory N losses (4 g) Add 2 - 4 g N if goal is repletion Convert to grams of protein (x 6.25) For neutral nitrogen balance UUN + 4 = g N g N x 6.25 = g protein For positive nitrogen balance add 2 UUN + 4 + 2 = g N
