Vigilance is the essence of nursing care and is composed of 2 types of nursing diagnoses,
central and surveillance. Central indicates the need for the nurse to plan and implement
interventions for the achievement of outcomes. Surveillance recognizes patient risks that are
anticipated by the nurse. The nurse must remain ready to act at any moment.
Ensure clear information exchanged between healthcare professionals. Ineffective
communication, disruptive behavior, and hierarchy are challenges for patient safety.
Being able to work with multiple team members to care for a patient and being able to be a
patient advocate when safety is an issue.
Health care quality identifies the gap that occurs between ideal care and actual care delivered.
Quality improvement measures the variance in ideal and actual care and implements strategies
to close the gap. If quality is lacking, safety is a risk.
A part of nursing due to the risk of the job with the safety of the lives of others if the nurse is
incompetent or unprepared.
