MBA665 Metrolink The Metrolink incident was an effect of a combination of factors combining together. First, there is the human factor at play which is Mr. Sanchez. Sanchez had a history of poor behaviors as "he received five informal discipline letters for absences and failure to follow rules" which had also been counseled twice before over cell phone use (Steiner & Steiner, 2014). Working a split shift complicated the problem, as there was no guarantee that Mr. Sanchez rested appropriately between the two 4 hour shifts which could have created a lack of focus. The failure of the company to properly discipline the worker only increased the problems. For a career like a railroad engineer, there needs to be higher standards and methods in ensuring compliance with the various rules. Though Connex had faith in him, they also had a problem of having the ability to enforce compliance. This failure to create an enforcement solution allowed Sanchez to keep the habitual use of his cell phone while on duty.
