Subjective:The patient states she is a 42-year-old female being seen for a routine physical. Patient states she walks one mile daily and gets 8 hours of sleep each night. The patient reports frequent headaches three per week. The patient reports no history of head injury. The patient states she hasa history of depression but does not take any medications to manage it. The patient states that sheis not experiencing any pain, aside from the headaches. Patient states that she has not noticed anydischarge or pain in the nose. She has not noticed any lumps bumps or lesions on her neck or anysoreness in her throat. Objective:The patient nose is symmetrical; there is no noticeable deviation in the sep-tum.
