Recognize best practices in developing data collection forms Data Confidentiality (for Human Participants) HIPAA : Health Insurance Portability and Accountability Act of 1996 Protects the privacy, security and confidentiality of patient data (Protected Health Information: PHI); also applies to research. Office of Data Management & Biostatistics (ODM), Palmer Center for Chiropractic Research (PCCR), Data Security and Confidentiality Policies “Paper data collection forms are stored in a locked ODM cabinet in the Data Core Manager’s office. Keys to this cabinet are held by the Data Core Manager, Data Managers and Data Entry Clerks. The Data Core Manager’s office is unlocked only during office hours. Data collection forms are not removed from the ODM offices until a study is complete, at which time the forms are filed in a data storage room that is fire- resistant, temperature controlled, and has a steel dead-bolted door. The PCCR computer server is stored in a locked, temperature controlled room with the other college servers. Back-up tapes of the network drive are produced nightly. The only identifier on a given data collection form is a unique research participation
