The Aging Spine Disc changes associated with aging: Boundaries between the nucleus and annulus become blurred Nucleus loses hydration/height Nucleus becomes more fibrocartilagenous Nucleus becomes dessicated and loses integrity as clefts form Radial and concentric fissures develop Annulus and lamellae become less organized Hyaline endplates calcify Nutrients to disc decrease Cross linking of collagen fibers increases Disc fragments common True disc herniation rare Other spinal changes in the older patient: Flattened joints of Lushka Thickening of the ligamentum flavum Decrease in general ligament elasticity Increased propensity towards sprains and strains Decreased muscle mass (spinal and general) Decreased bone density overall Increase in thoracic kyphosis Decreased range of motion Why does disc degeneration occur? Mechanical insult affects nutrient supply to disc Repetitive strains cause fatigue and failure in collagen network Single overload causes cytokinins to accumulate, radically increasing degeneration
