Name (print) _______________ KEY ________________________________ This exam consists of 12 pages (including this one).  Please be sure that all pages are included in your packet.  You may write on the exam –  it is yours to keep.  This page must have your name legibly  printed in the space provided above and your signature in the space provided  below.   Remove this sheet  from the exam and submit it with your scantron at the end of the exam.   If you fail to hand in this sheet, you will not receive a grade on the exam!!!! You must put your version number  (NOT THE EXAM NUMBER!!!!!  VERSION NUMBER ONLY!!!!!!) on the top of your  scantron  or it will not be graded .  You must also put your  TA’s name and lab date / time  on the top of your scantron, and be sure to  bubble in a number  (so you can identify your score on the grade roster).
