Unformatted text preview: Acute Asthma in Adults Epidemiology •4-5% of population affected •5-10% in childhood – most common chronic disease •7-10% of elderly – approx 70,000 admissions a year •2:1 Male: Female in childhood •74% increase from 1980-1996 •36% increase of ED visits 1992-1999, but decline in admissions Pathophysiology •Reversible airway inflammation •Decrease airway diameter •Increase in work of breathing •Causes • Allergic • Exercise • Meds Clinical Features •Dyspnea, Wheezing, cough •Accessory muscle use •Altered mental status •Silent chest •Pulsus paradoxus Diagnosis •Spirometry/Peak Expiratory flow • Mild 80% • Moderate 50-80% • Severe <50% •Pulse ox •Generally not needed: ABGs, Chest X-ray, CBC, EKG Treatment •B-adrenergic agonists • Aerosol best route – MDI or nebulizer equivalent routes • Albuterol • Xopenex •Anticholinergic • Atrovent •Corticosteroids – delayed effect, but may reduce admissions; used for prevention Treatment •Theophylline – no longer widely used •Magnesium – Useful for severe attacks in addition to other treatments •Heliox – Possibly useful for severe attacks, studies vary •Leukotienes – Maintenance use •Mechanical ventilation – for very severe cases ...
