Please provide the information requested at the top of this page, then initial the last five pages of the exam. As you do so, please verify that your booklet has 13 total pages. 2. On the left side of the Scantron answer sheet, bubble in your NAME, three digit DISCUSSION SECTION NUMBER, and eight digit STUDENT ID NUMBER. To ensure the scoring machine grades your exam with the correct key, enter the number shown above in the FORM column. On the right side of the Scantron, provide your signature, the course number, and the date. Under the heading "Other", enter your GSI's NAME. Your signature on the Scantron indicates your confirmation that you have not cheated, in any way, on this exam. Integrity and good academic conduct are expected of all UM students. 3. For each multiple-choice question choose the one best , most complete answer . There are 30 multiple-choice questions worth 2 points each. Use a No. 2 pencil to mark your answers on the Scantron answer sheet . Do not make marks on the answer sheet outside of the designated areas. Do not fold the answer sheet. There are 5 short answer questions worth 10 points each . You must answer only 4 of these; draw an X through the question you are not answering. IF YOU ANSWER ALL FIVE QUESTIONS, WE WILL GRADE THE FIRST FOUR AND IGNORE THE LAST ONE – THERE IS NO ADVANTAGE TO COMPLETING MORE THAN FOUR. These should all be answered in the space provided in your exam booklet. The exam has 100 total points. 4. Only multiple choice answers entered on the SCANTRON ANSWER SHEET will be graded. Questions with more than one answer marked will be scored as incorrect. There is no penalty for guessing. You may change an answer on the answer sheet by completely erasing the incorrect answer. Do not cross out an incorrect answer; the scoring machine will count it as marked. 5. At the end of the exam, turn in BOTH the ANSWER SHEET and ALL PAGES OF THE EXAM BOOKLET to YOUR GSI. You should have your M card available for identification purposes at the exam. 1
