Learning issue: Enzyme level for acute myocardial infarction Name: MH Cardiac enzymes: patients suspected with MI, obtain cardiac enzymes at regular intervals, starting upon admission and serially for as long as 24 hours Tropinin levels Troponin levels are the standard for defining and diagnosis Cardiac troponin levels have greater sensitivity and specificity than CK-MB levels Serum levels increase within 3-12 hours from onset of chest pain, peak 24 hours, and return to baseline 5-14 days Creatine kinase level Comprises 3 enzymes, creatine kinase with muscle subunits (CK-MM) found in muscle, creatine kinase with brain subunits (CK-BB) found in brain, myocardial muscle creatine kinase (CK-MB) found mainly in the heart. CK-MB was the previous standard, increases within 3-12 hours of onset of chest pain, peak within 24 hours, return to baseline 48-72 hours. Sensitivity 95%, with high specificity, but not has high as troponin. o
