Postherpetic Neuralgia Tshona Corbin What it is: Pain (either persistent or recurrent) in shingles-affected areas. It can remain even after the acute rash of a shingles eruption occurs. Pathophysiology: It seems that an imbalance from of loss of large inhibitory fibers, and intact or increased number of small excitatory fibers leads to hypersensitivity or increased spontaneous activity of pain receptors in the affected area. Namely, pain and temperature detection becomes hypersensitive due to formation of new connections involving central pain transmission neurons. Other patients may have spontaneous pain due to an increase in deafferented central neurons or the reorganization of central connections. Abnormal dorsal horns with hypersensitive inputs suggest that the pain is due to a combination both central and peripheral changes. Frequency: 1 month after the onset of shingles, will be seen in 9-14.3% of patients. At 3 months post-onset of shingles, will see in 5% of patients. 3% of shingles patients will continue to have
