Category Type Presentation Cause Diagnostics Treatment Other Macrocytic normochromic anemia Pernicious (Nicole) -Wt loss of 10- 15 lbs -Sore tongue -Paresthesias -Smooth tongue w/ loss of papillae (may be painful and beefy red) -Constipation or semi-solid bowel movements daily -Anorexia - Nausea/vomiting -Weakness -Clumsiness -Unsteady gait -Due to impaired absorption of vitamin B12 because of a lack of intrinsic factor in gastric secretions -Associated w/ gastric atrophy and a loss of IF production Peripheral blood smear: macrocytic anemia w/ mild leukopenia and thrombocytopeni a; oval macrocytes, hypersegmented granulocytes and anisopoikilocyto sis -Severe anemia can result in RBC inclusions w/ Howell-Jolly bodies, Cabot rings and punctate basophilia -MCV and MCH increased -MCHC is normal -Indirect bilirubin elevated -LDH increased -Serum Cbl levels decreased - cyanocobalamin - 100mcg IM/SC qDay x 7, then qOD x 7 doses, then q3-4 Days for 2-3 weeks, then qMonth -Multivitamin- 1 tablet PO qDay Folate deficiency (michael) Sore,beefy, red, swollen, and Inadequate ingestion of Serum folate -reference range Diet -Increase fruits
