Tinea Cruris (Jock Itch) Definition-a subacute superficial infection of perineum and inner thigh Etiology: Post-pubertal females who wear close fitting exercise clothes, panty hose Usually occurs in warm, humid climates Pathophysiology: caused by Tichophyton rubrum and/or Epidermophyton floccosum They produce keratinases, an enzyme, that invades the cornified cell layer of the epidermis Sx: Reddish brown symmetrical macules with well marked borders Infection typically spares penis and scrotum in males Fine, oozing vesicles may be present Not painful but tender with local pruritis Skin may turn to brownish color with chronic infection Differential Dx: Contact Dermatitis Erythrasma (Cornybacterium minutissimum)—this appears as velvety patch which turns a coral pink when viewed under a Wood lamp Seborrheic dermatitis Psoriasis Candida infection-this is more bright intense erythematosus
