Instructions for use of Exam Conflict Form 1. Open the form (found in Resources). 2. Type your name, RUID, and section number in the appropriate blanks. 3. Students with disabilities require special accommodation (extra time, scribe, quiet room, etc.), make sure you have submitted a letter of accommodation to us. 4. If the reason is not a conflict with a course or another exam, you must obtain appropriate documentation and scan it (or have an electronic copy). You do not need to scan class schedules, or other course syllabi. You only need documents relating to non-class schedule issues.

Remember, to qualify for a conflict final exam, you must have either: 2 exams schedule during the 9-noon time slot of 12/18; or 3 exams in a row (12/17 8-11pm, lab and 12/18 1pm-4pm; or 12/17 4-7pm, 12/17 8-11pm and lab). No other exam schedule will be considered as a conflict. 5. Once you have completed the form SAVE the file somewhere on your computer or thumbnail drive. 6. Go to sakai.
