As part of your work role you will have to provide individualised support to clients in accordance
with their individualised care plans.
These plans could be applied in the aged care, mental health, AOD, youth justice, childcare,
disability etc sectors of the community services industry and could be implemented in a
residential, home care or community care environments.
Write a 700 word report explaining how you would:

determine the support needs of clients

provide support services
o
monitor support activities
o
complete reporting and documentation

The first part of this report is to determine the support needs of the clients. Questionnaires and notes are
needed to analyze and understand what kind of support our clients will need. Before any communication, it is
important to review and get an overall idea about the basic information of the clients and their carers and
supporters.
