NURSING ASSESSMENT Regular atrial and ventricular rhythm Rate < 60 beats/ minute Normal P waves preceding each QRS complex Assess for: syncope, dizziness, weakness, confusion, hypotension, diaphoresis, shortness of breath or chest pain DIAGNOSTICS 12 lead electrocardiogram Holter monitoring Echocardiography Electrophysiology studies Coronary angiography MANAGEMENT For low cardiac output, dizziness, weakness, altered level of conscience, or low blood pressure follow ACLS protocol and administer atropine Evaluate patient for temporary pacemaker and later permanent pacemaker placement Administer medications as ordered Provide adequate oxygenation Monitor pulse rate and assess for s/s decreased cardiac output ETIOLOGY May be congenital May occur with anticholinesterase, beta blocker, digoxin, morphine use, carotid sinus massage, vomiting, suctioning, valsalva maneuvers, ocular pressure or pain, hypoxia, inferior wall MI.
