2 Preliminary Care Coordination Plan Preliminary Care Coordination Plan Analyzing COPD COPD is currently the fourth leading cause of death in the United States (Criner, R. N. et al. (2018). COPD is underdiagnosed, undertreated, and yet a largely preventable disease (Criner, R. N. et al. (2018). This disease affects millions of Americans daily, accounts for tens of thousands of deaths per year, and costs billions to the United States health-care system annually (Criner, R. N. et al. (2018). Tobacco dependence is the leading cause of COPD in the United States of America (Criner, R. N. et al. (2018). The daily symptom burden of COPD leads many to leave the workforce and become dependent on disability benefits (Criner, R. N. et al. (2018). Patients with COPD become more dependent on others for daily living and management of their chronic condition (Criner, R. N. et al. (2018). COPD prevalence increases with age and the life expectancy increases the more the treatment advances (Criner, R. N. et al. (2018). Women are more likely to be diagnosed with COPD and the races that it is most imminent in are White and African American women (Criner, R. N. et al. (2018). There are many things to analyze about COPD but these are some of the main facts and issues people face with it. In this preliminary care plan, it will discuss physical, psychological, and cultural needs of a COPD patient, and it will also discuss resources in the community that the COPD patients can utilize. Goals Established to Address COPD After COPD has been diagnosed the goals of treatment to address COPD would include
