Process Recording FormStudent’s Name:Date:3/5/20Client Age: unknownDSM 5 Diagnosis: unknownDescription of Interaction: I approached the patient and asked if she would further like to explain why she is in theplace she is today. The client stated that they would be more than happy to talk once we were out on the courtyard. Iwent out during the courtyard hour and sat down at a picnic table to discuss her past. This patient has a history ofgetting irritated quickly. Patient also has a history of depression and anxiety. Patient voiced to me that she sufferedfrom a lot of wishing bad upon people and was trying to work on moving forward in a positive direction. Sheactively is working on coping skills, but still has times of stress and frustration. Patient also was very hesitant toopen up to other staff, but felt comforted in my presence. I had had previous interactions with this client, discussingher tattoos and her ways of self expression. I was able to form a connection to her through this and the currentconversation stemmed from this.Environment: The environment was very relaxed. It was not formal with another therapist or mental health worker, itwas just between the two of us. We sat at a picnic table outside in the courtyard. There were other patients around, some adding to the conversation, but the patient was not bothered by people surrounding us. The patient was not athreat and kept a calm voice even when talking about life altering situations.Client goal/Purpose:Goal:Develop new ways to open up to people about sensitive topics and obtain different ways to manage her anger.Purpose: Patient lacks the ability to open up to staff. She feels like they will hold her past against her. She does nottrust people very easily and it is hard for her to speak on her traumatic past. She also gets upset very quick, as seenwhen she was not allowed into the closet (was not a staff member free to bring her) to get her essential oils foraromatherapy.Nursing Studentverbal statements(exact quotes)Client’s verbalstatements (exactquotes) /non-verbalbehaviorCommunicationtechnique used / analysis(rationale for using thistechnique)Student’s thoughts and feelings(any revision you would make ordo differently)1.“Do you feelcomfortablesharing some
