For patients who are under 60 years of age initiate treatment when SBP is ≥140 mm Hg. Patients of 18 years of age or younger with chronic kidney disease (CKD) should start treatment when SBP ≥140 mm Hg or DBP ≥90 mm Hg. Patients ≥18 years who suffer from diabetes should be treated to lower BP at SBP ≥140 mm Hg or DBP ≥90 mm Hg. Treatment goals: ≥60 years: goal SBP <150 mm Hg and goal DBP <90 mm Hg<60: goal SBP <140 mm Hg and DBP <90 mm Hg≥18 with CKD: goal SBP <140 mm Hg and goal DBP <90 mm Hg≥18 with diabetes: goal SBP <140 mm Hg and goal DBP <90 mm HgIn the African American population, including diabetics, initial treatment should include a thi-azide-type diuretic or a calcium channel blocker (CCB).
