JNC 8 Classification of hypertension: Normal: <120 & <80 Prehypertension: 120-139 or 80-89 Stage 1: 140-159 or 90-99 Stage 2: 160 or 100 In patients 60 years or older treatment should be initiated when patients have a systolic blood pressure (SBP) of 150 mm Hg or greater or diastolic blood pressure (DBP) of 90 mm Hg or greater. For patients who are under 60 years of age initiate treatment when SBP is 140 mm Hg. Patients of 18 years of age or younger with chronic kidney disease (CKD) should start treatment when SBP 140 mm Hg or DBP 90 mm Hg. Patients 18 years who suffer from diabetes should be treated to lower BP at SBP 140 mm Hg or DBP 90 mm Hg. Treatment goals: 60 years: goal SBP <150 mm Hg and goal DBP <90 mm Hg <60: goal SBP <140 mm Hg and DBP <90 mm Hg 18 with CKD: goal SBP <140 mm Hg and goal DBP <90 mm Hg 18 with diabetes: goal SBP <140 mm Hg and goal DBP <90 mm Hg In the African American population, including diabetics, initial treatment should include a thi- azide-type diuretic or a calcium channel blocker (CCB).
