Electronic Health Record An electronic health record (EHR) is a digital version of a patient’s paper chart that contains documentation of all health services provided, demographic data, diagnoses, treatment plans, test results, and other important data. The purpose of the electronic health record is to make patient records available instantly and securely to authorized users. This includes being able to share EHRs across different organizations. Information The type of information that is found in an EHR is a patient’s medical history, diagnoses, medications, progress notes, treatment plans, immunization dates, allergies, radiology images, laboratory and test results, vitals, and future appointments. Manages While there is controversy on who owns and manages the EHRs, it is actually the property of the provider and managed by government officials, healthcare providers, hospital staff, and patients can input certain information.
