2 Describe Evidence Based Medicine The conscientious, explicit and judicious use of current best evidence in making decisions about the care of individual patients. The practice of evidence-based medicine requires the integration of individual clinical expertise with the best available external clinical evidence from systematic research and our patient’s unique values and circumstances. Clinical decision making should be the integration of: - The best available evidence - Professional judgment to interpret the relationship between clinical trial participants and individuals in the pharmacist’s care. - Consideration of the individual patient’s characteristics and relevant clinical history. Formulate a research question PICO method is how the question will be broken down to identify all the required elements to help a patient with an illness. P Population or Problem or Patient. What are the characteristics of the population or patient? What is the condition or the disease? I Intervention or Exposure. What do you want to do for the patient or population? C Comparison. What is the alternative to the intervention? O Outcome. What are the relevant outcomes. Example Case Study: Your next patient is a 72-year-old woman with osteoarthritis of the knees and moderate hypertension, accompanied by her daughter, a lab tech from the hospital. The daughter wants you to give her mother one of the new COX-2 inhibitors. She has heard that they cause less GI bleeding. Her mother is concerned that the new drugs will mean more out of pocket costs each month. P 72 old women with osteoarthritis of the knees and moderate hypertension. I COX-2 Inhibitor. C Other Non-Steroid Anti-Inflammatory Drugs. (NSAIDS). O Less GI bleeding whilst maintaining pain control. Research Question Formulated: In a 72-year-old woman with osteoarthritis of the knee can COX-2 inhibitors use decrease the risk of GI bleeding compared with other NSAIDs. It must be defined on a well-known single disease. 3. The time of action must be observed. 4. The effect of the drug must occur constantly and not transient.
