Checklist for proposal outline A. You have just been designated fund-raiser for a small group of nurse practitioners (whether you are an NP or not) who plan on opening their own practice. Be creative on what the practice will be like and where it would be located. 1. Using the proposal outline in the lecture notes, create a grant proposal 1. What are the goals and objectives for the project? I’m opening a community services non-profit health clinic for those that in Waterville NY, which has a population of 1,682 people and is thirty minutes from an urbanized area. There are going to be five RNs, two nurse practitioners and one secretary. The clinic is for people in Waterville that don’t want to travel thirty minutes to get an annual check-up. We would like to open the clinic within six months as soon as we raise enough money for supplies in the building and get the approved raise in local taxes. 2. What organizations are involved?
