Admission Orders Pneumonia Primary Diagnosis: hospital-acquired pneumonia Status/Condition : acutely ill, stablizing Code Status: Total support Allergies: no known drug allergies Admit to Unit: med/surg floor Activity Level: ambulate with assistance as tolerated Diet: regular diet IV Fluids: NS at 75mls/hr, until patient is able to eat & drink > 50% of meals, then d/c Critical Drips: Zosyn 4.5gm IV q 6 hours; tobramycin 5mg/kg IV q 24 hours; Vancomycin 15mg/kg IV q 12 hours; pharmacy to dose abx based on patient’s weight, pharmacy to dose & hold Vancomycin based on trough values
Respiratory: Oxygen up to 6L/min continuous via nasal cannula to maintain O 2 saturation > 92%, encourage pulmonary hygiene, call MD if O 2 needs exceed 6L/min or if unable to wean O 2 and maintain O 2 sats > 92% Medications: Hold ciprofloxin Rivaroxaban 20mg po q day with dinner
