History of patient illness:
 The patient had her regular check
up with attending physician, BP
taken 170/100, no headache, no
chest pain, patient has been
complaint with her maintenance
medications and was advised for
admission.

Social history:

Smoke 5 pack / yr.
 Alcohol Use: Occasional drinker

Family history:

Patient family disease
backgrounds are hypertension,
diabetes mellitus.

Medication history:

Current Prescription Medication
 Amlodipine (Amvasc) 5mg OD, for
hypertension

Glimepiride (Norisec) 2mg ac-bf, for type 2 DM
 Gabapentin (Neurontin) 300mg OD, for the
management of postherpetic neuralgia in
adults
 Insulin Aspart(Novorapid) 8units TID, for type
2 DM

Pioglitazone (Piozone) 30mg OD, for type 2 DM
 Clonidine (Catapres) 75mcg SL prn over
150/100, for hypertension

