“Captain of the Ship” Project – Schizophrenia Spectrum and Other Psychotic Disorders Walden University NURS 6670, PMH Nurse Practitioner Role II: Adult and Older Adult January 31 st , 2021 Schizophrenia is one of the most common severe mental disorders, and about one person in 100 will develop the condition during their lifetime (Sadock, 2014). It usually begins before
age 25 years and persists through life (Sadock, 2014). Signs and symptoms for schizophrenia differ in clinical presentation and 'include changes in perception, emotion, cognition, thinking, and behavior' (Sadock, 2014). Schizophrenia is equally predominant in men and women, although the men have an early onset. Despite how severe the disorder is, only half of the patients diagnosed get treatment. 15-45% of people diagnosed with schizophrenia make up the homeless percentage in the U.S. (Sadock, 2014). HPI and clinical impression for the client D.S is a 54-year-old A.A. male diagnosed with Schizophrenia, Hypertension, and Diabetes. D.S. had several incidents of physical aggression and uncontrolled behaviors and has been hospitalized many times. He complies with his medications and doctors’ visits, though sometimes he gets psychotic and refuses to take his medications. D.S. lives with his aunt Perlin. D.S.'s sister is fearful of him because, in the past, D.S. hit her with a hammer causing her to be admitted to the hospital. D.S. comes in today, and he looks 'high.' His eyes are wide and moving around rapidly as if he is searching for someone. He is pacing in the waiting area, jabbering about how the police should arrest his neighbor because he believes his neighbor stole all his C.D.s and records. He also states that the neighbor assaulted him the day prior and called the police at home. His aunt states this is not the case as the neighbors’ house is up for sale and thus
