3FLAGS1.A DA Form 268 (Report to Suspend Favorable Personnel Actions) is prepared to initiateor remove a Flag.A.Initiation: When a flag is generated the S1/PAC should notify the following:a.Security Manager – If the reason for flag affects the soldier’s security clearance.b.PSB – The soldiers Military Personnel File can be moved and maintained in arestricted access area.c.Reassignments – If the soldier is on an assignment the flag will prevented thesolider from being reassigned.d.Promotions – If the solider is on the promotion standing list.The DA Form 268and a memorandum requesting the removal of the soldier from the promotionstanding list must be submitted to the PSB.B. Removal:When a flag is removed the same notification process is necessary.2.AAA-095 (Suspension of Favorable Actions Management Report)a.The AAA-095 is printed by the unit S-1/PAC and is reviewed and signed bythe commander of each unit.It must be submitted to the PSB by the tenth ofevery month.Each Soldier on the report that has had a flag initiated orremoved must have a DA Form 268 attached to the AAA-095 initiating orremoving the flag.b.If the soldier is on the promotion standing list a memorandum should beprepared requesting removal from the promotion standing list. Thememorandum should specify the reason for the flag and the supportingdocuments should be attached.
4BAR TO RE-ENLISTMENT1.The BN S1 is required to forward the Bar to Reenlistment certificate to the PSBwithin 5 working days of the Battalion Commander’s signature.2.The BN S1 must ensure the unit Commander reviews the Bar certificate every 90days.The reviews must be turned into the PSB within 5 working days after thereview date.3.Reviews are 90 days apart; the soldier can only have 2 reviews.If the soldier’ssecond review is denied for removal, the bar must be lifted and then initiate chapterproceeding at the discretion of the commander.4.The PSB will receive the Bar, review the bar, and ensure it is filled out correctly.
