Individual Wellness Plan Instruction Sheet (20 pts) Name: Zach Frank Section # 11 Date: 10/23/07 Instructions : Refer to Concept 1, concerning the five dimensions of wellness. Refer to Concept 6 for information on writing goals. Your task is to construct one long-term goal (achievable in months/years) and one short-term goal (achievable in days/weeks) for each dimension. All goals must satisfy three requirements: 1) Specific (what exactly am I going to do), 2) Measurable (how do I know when I’ve done it), and 3) Contain a deadline . Beginners should focus on behavioral goals. Those with more experience may want to use outcome goals. Those with high performance levels in a certain area should consider maintenance goals. Format : Set up your own grid to look like the one below or use this one to expand and type in your goals. Your final submission should fill an entire page, be readable, and contain your name, section number, and the date on the top. Dimension Long-Term Goal Short-Term Goal Physical I am underweight and not to strong, so my goal is to gain 5-10 pounds of muscle by the new year. I will do pushups, sit-ups, and pull-ups at least 4 times a week. And try and make it to the gym at least twice a week. Intellectual I will get above a B in all my classes for the Semester I will sit down and do homework or study for at least and hour a night. Emotional I get stressed out a lot by being busy, so my goal is organize my things better and not procrastinate on school work for the rest of the year. I will buy a planner and Calendar within
