The first one is known as Antisocial Personality Disorder, a personality disorder in which the person exhibits a lack of conscience for wrongdoing, even towards friends and family members. Additionally, the person suffering from this disorder may also be aggressive or ruthless. The Joker shows this disorder basically throughout the entire movie. It is not necessarily against his family, but against the entire city of Gotham, including Batman. He nearly destroyed half of Gotham in the process, but that was the extent of what he wanted to do, showing lack of conscience for wrongdoing. He did not care about anything else, other than chaos.
