HEALTH PROMOTION PROJECT 2AssessmentThe client I chose for the Senior Health Promotion Project was a 67-year-old Caucasian woman, who has been married for 43 years, and has four grown children and four grandchildren. Both of her parents passed away in their 90’s and brother died of esophageal cancer in his 50’s. Her and her husband live in their home, located in rural Northern Neck of Virginia. The client is a retired home-health nurse of 20+ years. In the last three years she has assumed the primary role of caregiver for her spouse, who suffers from multiple cardiac conditions. She does the decision-making in the family and makes all her own personal healthcare decisions. She is proficient in the English language and did not need any assistance understanding the assessment questions. She did, however require glasses to read the material provided. The client viewed herself in a fair/good state of health. She recognized; having a balanced diet, exercising, taking medications as ordered, decreasing stress and getting adequate rest, as affects to healing and recovery. Her way of promoting personal good health is by “eatinga fairly well balanced meal and not smoking”. The client defined good health as “not having to go to the MD frequently, maintaining a well balanced diet and exercising”. She tends to eat mostly American cuisine, but her favorite foods are sweets and carbs! She has no dietary restrictions, however does suffer from a narrow esophagus, resulting in reflux. Current medical conditions she listed were osteoporosis, depression, high cholesterol and gastric reflux. Medications are located on attached document. Past medical history of kidney stones and surgeries including; tonsillectomy, cesarean section x3, double knee-replacement, removal of parathyroid, TNA, eye surgery, fractured arm repair, and back surgery.
