1 Module 1 Discussion Board The interview to the patient includes all the fundamental information that could help the healthcare provider to understand the patient as a whole. Information obtained from the interview includes the patient’s demographics, chief concern, history of present illness, family history, past medical history, etc. After establishing a rapport with the patient, the provider could educate him on risk factors that are identified in the interview ( Dang et al., 2017). Narrative The patient is a 60-year-old male that identified himself as white but Italian descent. He is married and had two daughters. He currently lives in Plano, Texas. He is a research scientist currently employed by a university. He was referred by his friend because he is seeking a primary care provider (PCP) near his living house. The highest level of the patient’s education is a PhD’s degree. He is a research scientist, and has been working as a scientist for over 25 years. The patient has been married to his current wife for 32 years and states a good relationship with his wife. His both children are no longer live with him. He denies any recreational drug use and no smoking. He currently drinks one to two cups of coffee a day, drinks three to five alcoholic beverages a week. He likes hiking and exercises two to three times weekly, including walking after dinner. This history is obtained from the patient as he is considered a reliable historian.
