diabetes mellitus type II. One sister has ovarian cancer stage III. Other sister has no other health issues. Both parents are deceased with no significant health histories. He has two daughters, one deceased at age 47 from poly arteritis nodosa Social History: Patient is a retired military accountant, he has been married for 54 years, and lives in his own home with his wife. He is not sexually active. He has one daughter living and five grandchildren. Patient works a job at a golf course to keep himself busy after retirement. Review of Systems : General: Mr. Hamlin looks healthy and like he eats well. He is pleasant and answers questions cooperatively. He appears to have good hygiene practices. The patient is a good historian.
INTEGUMENTARY 4 HEENT: The patient states he has o vision or hearing changes. The patient does wear glasses, and his last eye exam was 1 year ago. He reports a history of glaucoma. The patient has had no recent infection of the ears. The patient reports his sense of smell is intact. His nares are bilaterally patent with pink mucosa. The patient's last dental exam was 11/2019. The patient wears dentures. The patient denies any difficulty in chewing or swallowing. Neck: The patient's neck shows no sign of injury. He has no complaints of pain. The trachea is midline. Breasts: The patient has no history of masses, rashes, or lesions Respiratory: The breath sounds are clear, but diminished in the bilateral lower lobes. CV: Patient states that he has no chest discomfort. Has not history of heart murmur or arrhythmias. The patient's last cardiac workup was 05/2016 GI: The patient does not complain of any nausea of vomiting. Last bowel movement was yesterday and was formed. Patient does not complain of abdominal pain. GU: Patient does not complain of urinary incontinence, frequency, or urgency.
