In addition to accreditation, The Joint Commission offers certification to facilities who further their goal in providing high- quality patient-centered care. Certifications are available for many chronic conditions, such as heart failure and stroke ("Hospital Certifications", n.d.). Balik, Conway and Watson (2011) indicate that when we involve patients in their care, outcomes are improved when treating chronic conditions. Reimbursement Healthcare facilities must meet certain requirements for reimbursement of services. With the introduction of the Affordable Care Act (ACA) in 2010, reimbursement is based on the value rather than the volume of care. Value can be assessed through patient satisfaction scores as compared to other facilities. After discharge, the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) randomly surveys and scores how the patients perceived their care. These scores are compared to other facilities and reported for the general public to view (Sullivan, n.d.).

ORGANIZATIONAL LEADERSHIP 4 With these changes, patient-centered care is now a must if the facility expects the Centers for Medicare and Medicaid Services to provide full reimbursement for services. Pay-for- performance programs act as either a reward or punishment for provided care. When agreed- upon quality measures are met, a bonus is awarded. Financial penalties are imposed when specified goals or cost-saving measures are not met. For example, treatment for hospital- acquired pressure sores and infections acquired from the use of an internal urinary catheter is not reimbursed by Medicare (James, 2012). The healthcare setting assessed for the Patient and Family Centered Care (PFCC) Organizational Self-Assessment Tool is Saint Thomas Midtown Hospital. Healthcare Setting Saint Thomas Midtown is an acute care hospital facility located in Nashville, Tennessee that has been serving its community for over one hundred years. It is the largest non-profit hospital in middle Tennessee with 683 beds, 2800 medical staff members and offers greater than thirty medical and surgical specialties. St. Thomas Midtown is part of Ascension, which is the nation’s largest Catholic not-for-profit health system. Midtown has been certified by the Joint Commission as a Primary Stroke Center, Advanced Chest Pain Center, and also received the Gold Seal of Approval for Total Hip and Knee Replacement. Blue Cross Blue Shield of Tennessee and Blue Care Network named Midtown a Blue Distinction Center in bariatric care ("Saint Thomas Midtown Hospital", n.d.). Strengths and Weaknesses
