The assigned case manager works to coordinate the services and assure the success of the plan for the patient. Serves seriously and persistently mentally ill who require many supports and who have not been adequately served by traditional mental health programs. Rochester Regional Health -GMHC, RMHC Strong Behavioral Health Unity Health System Comprehensive Care Initiatives Serves individuals with serious mental illness and difficulty accessing or maintaining mental health services (includes the homeless and incarcerated populations as well as those with multiple psychiatric hospitalizations/arrests or a history of substance use) Strong Ties Assertive Community Treatment (ACT) Program Rochester Regional Health Behavioral Health of Rochester General- MICANET Community Residence Programs (Supported Housing) Provides a homelike environment, room, board, and supervision. This level of care is aimed at helping a patient learn the necessary skills to live independently and attain emotional stability. De Paul Community Services East House Housing Options Made Easy Ibero American Action League RPC Community Residence Program* Single Room Occupancy Programs Provides room, board, and on site psychiatric services designed for individuals whose needs encompass supervision and some care for psychiatric problems. Supported Housing DePaul Community Services Family Care Homes Provides mentally disabled persons a home like atmosphere by living with a family in community with medical/psychiatric follow-up care and supervision by and standards set by state. N.Y.S. Family Care Program (RPC) Home Care Provides short-term or long-term assessment and monitoring of mental status, medication compliance, etc. with home health aide services as needed.
