Investigators traced the source of the bacteria phials of fentanyl which has the potency of being a narcotic (Cheung, 2010). The investigators found that a therapist who was an addict had extracted the fentanyl. He had then replaced the fentanyl with water which had been contaminated with the bacteria. In this product tampering case the individual had not intention of damaging the company’s image or gaining financially. He just wanted to satisfy his addiction.

CRISIS EXPLANATION In the case involving Cabot Nutrition, an employee was responsible for product tampering. 1 The tampering occurred during production. It could be intentional since infant formula is made from measured proportions of chemical compounds. Anyone working on the production clearly understands the impact of using the wrong proportions of the compounds. 2 From the facts it seems that employee worked alone since had it been with the other employees the other two employees would not have used the product that particular day. 3

CRISIS MANAGEMENT TEAM FUNCTION The function of the Crisis Management Team would be: i. Determining the problem areas ii. Discuss with the firm’s employees to find solutions for the areas of concern iii. Come up with a crisis management plan iv.
