Lastly, it would add an extra layer of protection for medication administration as wrist bands and medications would need to be scanned for verification purposes. An important aspect of change is being able to track or measure the effectiveness of the changes made. All the recommendations provided by the author would be measured by an interview of the employees conducted after the changes have been implemented for at least three to six months. For example, if Nurse Jackie decided to move into a larger home, she would be able to answer questions pertaining to how the changes have impacted her personal life and her business. The same measuring tool would be used to measure the effectiveness of employing an additional worker as well as utilizing an electronic medical records verse handwritten documentation. CONSULTATIVE CHANGE RECOMMENDATION PAPER 12 Being that the facility requires approval through the local Department of Social Services, random visits are required to ensure the facility is meeting state and national standards for Senior Home Care facilities. There is a checklist of items used to evaluate the safety of the patients, the care they are being provided as well as the cleanliness of the home. Based on these visits, recommendations are made that are required to be implemented within a certain timeframe and then a follow up visit is scheduled. These visits are extremely important as it will determine if the facility can remain open and it further ensures the safety of the patients as data is compared to the national benchmark of performance. Additionally, The Consumer Assessment of Healthcare Providers and Systems, or CAHPS, survey is the most ideal national benchmark for measuring change effectively as it has a survey specifically for Home Health Care Agencies such as Grandma’s Hands, LLC. CAHPS surveys are designed to capture patient’s reports and ratings of their experiences with health care services (2006). It is applicable nationwide, standardized and free to the public. The Agency for Healthcare Research and Quality’s website indicates that the CAHPS Home Health Care Survey asks patients who receive home health care services about their overall experience and the care they received from the agency (2014). Based upon the responses provided by the patients, measurements are comprised that focuses on the care of patients, communication between providers and patients, specific care issues, rating of care provided by the agency and willingness to recommend the agency to friends and family (2014). It was recommended by the author that Nurse Jackie provides this survey to all her patients in order to identify areas of opportunity, increase patient safety, quality care and patient satisfaction.
