See attached “Patient-and Family-Centered Care Organizational Self-Assessment Tool” (PFCC) for a healthcare organization. Domain Strength Weakness Leadership/Operations SHMC leaders commit to patient- family partnership and perform hospital rounds in which they speak to patients/families about their care. Although the leaders support patient family centered care (PFCC), and patient outcomes and safety are always at the center of plans and goals, PFCC is called out only in a few policies and procedures. I could not find any evidence of patient/family involvement in governing board activities. Mission, Vision, Values The Patient Bill of Rights is very patient/family friendly and centered around the patient/family. The core Values surround the needs and uniqueness of every individual and a partnership. PFCC is not mentioned in the Mission Statement. The Mission statement talks about caring for the poor and vulnerable but does not specify a partnership with patient/families. Advisors No strengths noted. This is a weakness as I was not able to find any evidence of patient/family members serving on hospital committees, safety rounds or advisory councils. Quality Improvement Patient survey results, recommendations and calls to customer service, are taken into account for strategic /operational goals. Improving customer service and patient outcomes are always a key goal. Patients and families are interviewed by leaders during leaders’ daily walk-arounds. There are no patient/family advisors on QI teams and they do not currently participate in quality/safety/ risk meetings nor do they attend IHI, HPSF type meetings.

INTERPROFESSIONAL TEAM DEVELOPMENT 6 Personnel Staff/physicians are encouraged during training to include patients and families in shared decision making. On the nursing units, bedside report occurs with the patient and families encourage to participate. PFCC in not mentioned in job descriptions or policies/ performance appraisals. Patients/ families are rarely part of interview team and they are not present at new employee orientation. Environment & Design The hospital continues to remodel rooms with patients and families in mind, and do gather input from patients/families. The hospital is making rooms bigger with more space for visitors and chairs that convert into beds. Patients and families are encouraged to participate in bedside shift reporting. Grand rounds with multidisciplinary team members do occur at the bedside of patients with patients and families involved. When a larger space is needed, the unit conference rooms are used. No weakness identified. Information/Education The hospital does have a web portal which allows patients and families to access a wide variety of educational materials and resources for free as well as sign up for classes, seminars, etc.
