stating the information, the pre-op nurse and I confirm the information. The pre-op nurse will also report any surgical or past medical history to me that the patient confirms. After surgery I use the Intershift Report communication tool. ISHAPED cannot be used because the patient is under the influence of anesthesia. Intershift Reporting is used between the PACU nurse and myself. Along with giving the PACU nurse the same information I received from the Pre-op nurse, I also include medications used, dressing that were applied, where the surgical wounds are located and any pertinent information that happened during the surgery. When healthcare team members do not communicate effectively with one another the delivery of a patient’s healthcare will be hurt. Dysfunctional communication patterns

among the healthcare team are not collaborating as a team. Collaboration requires individuals to communicate information about a patient in their own area of expertise. In order to achieve effective communication, each team member in the healthcare must be willing to participate equally in a patients’ care. Without collaboration, the responsibilities for a patient are not shared and insight from other disciplines is not achieved, leaving holes in a patients’ care. Collaboration among the healthcare team is essential for nurse to provide high quality healthcare to patients. (Kearney-Nunnery, 2016) Another source of dysfunctional communication is the use of jargon and vocabulary. Communication is not successful if another healthcare team member does not understand words or phrases being used. (Kearney-Nunnery, 2016) As nurses, we must remember that we are communicating with a wide variety of disciplines and we must use language that is understood by everyone.
