Rural areas have few hospitals, this might be another barrier because the child might not have had all necessary vaccinations dueto limited accessibility to health centers not neglecting the financial capability of the parent (Kashala-Abotnes, & et al., 2016).To organize my obtained data, I will use the SOAP approach. S for subjective( information gather from child and parent), O for objective ( which is my principal observation that I got fromthe patient and parent), A this is the in depth assessment which include both S and O,and lastly Pis the plan .
