Activity Identify five barriers which may prevent the delivery of effective person centred care. It is very important to have the support of people within your organisation for the delivery of high quality service. Workers in positions of authority will ideally lead by example and communicate the benefits to others. However, there are several barriers which may prevent the successful provision of care. The barriers include: Social, religious and cultural beliefs which prevent the effective delivery of care Communicative problems Lack of finance Insufficient space in the care environment Fear of making changes because of scrutiny by supervisors and managers

Activity Identify three ways of evaluating the person-centred care procedures that are carried out within your organisation. These are ways of evaluating person-centred care: Self-assessment Observation Client feedback Detail the ways in which you should review procedures in accordance with the changing aspirations, needs, and preferences of people with disability. The care that you provide should be specific to the requirements of each disabled person. The carers should follow common policies and person-centred procedures. The disabled people should be given the greatest opportunity to make personal progress and meet their goals. They should be treated with dignity, compassion, and respect. It will be necessary to work together with the disabled people to identify the most appropriate forms of treatment and individualised support. You should arrange regular meetings and ask them for feedback about the care process. It is quite possible that additional best practices will be identified over time. Your organisation should remain flexible and implement any changes necessary for individual satisfaction.
