MUSCLE BREAKDOWN FOR SURVIVAL AFTER INJURY - A lot of research compares Q in young people vs. elderly, b/c we're an aging population and there’s muscle loss with aging: sarcopenia . o Sarcopenia = risk in morbidity and mortality b/c muscle = body strength = you don't walk well and risk of falling. o Losing muscle mass also results in less protein mass to help recover from injury, infection or other metabolic stresses such as heart attacks or cancer So the 2 major killers are in part related to how much muscle/protein mass of the body at the time of this injury - People with lesser amounts of muscle/ protein mass ( i.e.- elderly) don't fair as well as those with more ( i.e.- younger people) with the same kind of injury because younger people have more protein in the body. Why? o **During serious injury muscle tissue becomes less needed for survival, so the body will raid muscle ( breakdown rate and of synthesis of muscle protein) to use up those freed AAs to syn proteins better suited for survival

SFLORES_3/10/2014 18 Breakdown rate of SM is accelerated under injury or other metabolic stress to provide more AAs to synthesize proteins needed for survival until its healed up The body then recycles all the AAs that you’re releasing from muscle (~80%) to syn proteins to necessitate your recovery - Also been shown that people who are underweight (don't have much protein mass of the body) have poor surgical prognosis for survival from burn injuries (one of the worst traumatic injuries you can get and accounts for the most muscle prot breakdown for survival prot syn) o It's the healthy muscular male who after serious injury excretes the most N and it's the elderly pers on who’s sarcopenic who gets an injury and excretes very little N. But the older person has a lower chance of survival i.e.- a football player who breaks his leg will lose more N (as shown in urine), but will also gain it back faster than an elderly person who breaks their leg or a person with not that much muscle mass. It may be that the body can raid and catabolize the muscle mass of someone with a lot of muscle to get the AAs needed for survival proteins and still have enough mass left over in the body for survival 28. N BALANCE AFTER BURN INJURIES - The amount of breakdown of SM (the primary source for all these AAs) is in direct proportion to the severity of the injury o Burn injuries produce the greatest loss (~ ½-2/3 of the body) & will elicit the greatest weight loss and the most amount of SM being broken down Negative N balance occurs during injury, b/c although you re releasing all those AAs from metabolism of SM and making survival proteins there’s still going to be a lot of AAs that aren’t being utilized and so you have to oxidize them and there ’s a tremendou s in urea and ammonia after injury -
