NECK/LYMPHATIC _____1. Inspect the neck _____2. Palpate neck/face lymph nodes _____3. Auscultate bilateral carotid pulse CARDIOVASCULAR/LUNGS ____1. Auscultates lungs in anterior and posterior fields ____2. Auscultates heart sounds in all 5 areas BASIC NEUROLOGICAL EXAM (OOOTTAFAGVAH: 1. Assess Cranial Nerves 1- 12: _____Cranial Nerve I (Olfactory):Have patient occlude one nostril and close their eyes and ask patient to identify smell (alcohol); repeat with other nostril. _____Cranial Nerve 2 (Optic): Check central vision with Snellen Chart (assess each eye separately) and peripheral vision with Visual Fields Test. Check pupillary response to light [PERRL]. _____Cranial Nerve 3 (Oculomotor), Cranial Nerve 4 (Trochlear) & Cranial Nerve 6 (Abducens):Test for conjugate gaze with extra ocular movements (Six Cardinal Fields of Gaze Test: “Star” or “H”pattern) _____Cranial Nerve 5 (Trigeminal): Test both Sensory - Patient closes eyes when checking light touch to face of the forehead, cheeks, chin, and nose. Motor portions –Ask patient to clench teeth and move jaw side-to-side (palpate while patient does this). (Not testing corneal reflex which is CN 5 sensory). _____Cranial Nerve 7 (Facial): Testing Motor only –Ask patient to smile, frown, raise eyebrows, puff cheeks, and pucker lips. _____Cranial Nerve 8 (Acoustic):Whisper Test (Whisper and patient repeats words back or rub fingers together and ask patient if heard them). Test one ear at a time.
