patient looks younger than his stated age. The legs and arms were symmetrical and there was no obvious anomaly. The patient is overweight, overweight defines as BMI=25.0-29.9 (Flegal, Kit& Graubard, 2014, p.288). He currently weighs 97 kgs and his height is 184cm (BMI = 29). He looks a little bit fat and there was no obvious muscle. His body movements were smooth. He could control himself to use eye contact and some body language to express his thoughts. He also could answer questions clearly. The patient looks tidy and clean but tired and upset. He can’t sleep well and don’t have an appetite because of the persistent pain. The patient didn’t eat breakfast and the last meal was on yesterday. Mr Sanderson was frowning, groaning and even could not pay attention during the assessment process. The patient also looks anxious and nervous because he was worried about his health condition. Vital signs measurements include body temperature, pulse, respirations, blood pressure and oxygen saturation (SpO2) ( Estes, Calleja, Theobald& Harvey , 2016, p. 142). Vital signs are the important parameters to nurses to identify patients’ health conditions during the health assessment, ongoing monitor them is one of nursing responsibility ( Crisp, 2017, p. 532). During the health assessment of Mr Sanderson, his pulse, respirations and Oxygen Saturation were in the normal range. His pulse was 91/min, the respirations was 19/min and the Oxygen Saturation (SpO2) was 97%. However, he got a fever and his blood pressure was also higher than the normal range. His

tympanic temperature was 38.4 and the normal tympanic temperature of adults is 36.7-38.0°C. The blood pressure of the patient was 145/95, slightly higher than the normal range. For adults, the normal range of systolic should be 90-140 and the diastolic should be 60-90 ( Estes et al., 2016, p. 143-151 ). The information gathered from the general survey could provide health care professionals with the important information and the data of the general health status of a patient (Estes et al., 2016, p. 137). In pain assessment part, he said the abdominal pain happened suddenly from 5 days ago. The pain only occurs in his abdomen. He also described the pain scale as 7 out of 10 when he moves.
