Works and lives in the City. Lives with significant other in an apartment in a three-story building. Review of Systems: General: No fevers, chills, or fatigue noted. Patient denies any recent significant weight changes. HEENT: Wears glasses with no recent vision changes or discomfort. Last ophthalmology visit 3 months ago. No changes in his hearing and no other symptoms present. Last hearing test performed 6 month ago. Denies any current upper respiratory infections including sinusitis or rhinitis. Last dental visit 1 week ago. Neck: No trauma or injury in the past. No impairment in movement. Breasts: No pain or skin changes to the chest. Respiratory: Denies cough, dyspnea, or sputum production. Cardiovascular/Peripheral Vascular: Denies palpitations or chest pain. No swelling noted. Gastrointestinal: No tenderness. Regular bowel pattern. No nausea or vomiting. Genitourinary: Normal urinary pattern. No incontinence, oliguria, dysuria noted. No history of STDs. Musculoskeletal: No history of fractures or musculoskeletal disorders. Psychiatric: Well-managed bipolar disorder. No suicidal history. Denies sleep disturbances or recent anxiety or depression. Stressful work environment. Neurological: No coordination issues. Skin: No previous skin issues. Family history of psoriasis. Recent skin changes noted to elbows and knees. Hematologic: No hematologic complaints or history. Endocrine: No endocrine complaints or history.
