There is also 12.5% of the Delaware population living in poverty (United States Census bureau, 2019). ChristianaCare serves all of Delaware and that includes a mix of urban, suburban and rural areas (Delaware Health and Social Services [DHSS], 2016). The main health risks of Delaware include: binge drinking (18%), cigarette smoking (25%), lack of exercise (27%), overweight/obesity (59%), lack of health insurance (10%), and they do not get the recommended daily serving of fruit and vegetables (81%) (Delaware Health and Social Services [DHSS], 2004, para. 1). The main diagnoses in Delaware are: Hypertension 34.9%, high cholesterol 34.6%, arthritis 25%, depressive disorders 19.7%, asthma/COPD 10.6%, diabetes 7%, skin cancer 6.9%, heart disease/heart attack 6.4%, stroke 3.4%, and kidney disease 3% (Delaware Health and Social Services [DHSS], 2019). These diagnoses and social determinants weigh very heavily on the community and the facility. The lack of exercise and lack of fruit and vegetable intake leads to obesity and can facilitate some disease such as heart disease, hypertension and diabetes. This shows a large component of an effective health system in this community needs to have a focus on every stage of health to really impact the overall state of the community. 5 CONSULTATIVE CHANGE RECOMMENDATION The nurse leader interviewed was the Chief Nursing Officer (CNO) for acute care in ChristianaCare. In this role she leads all of nursing practice and operations for acute care within the hospital system (Cathy Fernald, personal communication, February 14, 2020). She helps support exceptional experience, which encompasses patient and caregiver experience, optimal health, which looks at nurse sensitive indicators and preventing undue harm to patients, and organizational vitality, this is remaining fiscally and financially sound (Cathy Fernald, personal communication, February 14, 2020). She works with other nursing leaders to enhance personal development for all caregivers. She also helps with education and recruitment of talent (Cathy Fernald, personal communication, February 14, 2020). In this role she reports to the Chief Nursing Executive (CNE), senior leaders and corporate executive team. She serves physicians, nursing leaders, nursing frontline staff, nursing support, and patients/families (Cathy Fernald, personal communication, February 14, 2020). She
