Another barrier include insufficient or not having any insurance, which often leads to not having a PCP and only been seen at the ER in extreme cases; ultimately leading to their needs not being met, delay in delivery care, failure to get preventative services and hospitalizations that could have been avoidable. Another barrier include lack of culturally competent providers. This is a community that is very diverse and it is upmost importance that healthcare providers are culturally competent when providing care. Although there are a number of services provided for the community, often times the wait times is long, which deter the residents from waiting to be seen. “The community suffers from a host of health issues, including disproportionately high incidences of obesity, diabetes, asthma, cardiovascular disease, mental illness, and other chronic health conditions” (Pasquel.2015). The Bronx has a number of community based resources that exist in improving the health of the community. The services that offered in the community are services geared at treating, education and preventing the health issues most associated within that community. Some of the opportunities include: smoking cessation programs, as we all know the consequences smoking has on one’s health; preventative care and disease management (primary and secondary interventions), these programs are aimed educating the patients on their health and diagnosis and teaching them how to live and combat whatever illness they are dealing with to live their best life; prevention of HIV and STD’s. Although health insurance remains a barrier in this community, they have a number of free clinics located not only in the Bronx but throughout NYC that can be utilized. Food pantries and farmer’s market are providing this community with healthy nutritious food options at no cost to the individuals. Financial assistance programs are located throughout the Bronx that offers some form of financial assistance; Housing services that assist residents in finding affordable housing and being placed in healthy living environments;
COMMUNITY ASSESSMENT Educational services, and vocational and job training skills, in an attempt to better the individuals within in the community; Family and individual counseling; Community outreach, which offers the following services to the community: syringe exchange, crisis intervention, career development, fitness classes, at risk youth assistance, employment referrals to name a few. Healthy school is also another opportunity that exist within the community. The purpose of health schools is to encourage good health both inside and outside the classroom, while increasing opportunities for physical activity and healthy eating.
