- Bentiromide test or BT-PABA excretion action of chymotrypsin. After ingestion of 0.5g of BT-PABA, chymotrypsin cleaves the benzol-tyrosyl peptide bond and releases p- aminobenzoic acid which is absorbed and conjugated to glycine in the liver and excreted by the kidneys. Amount of p-aminobenzoic acid is measured in the urine collected over 6hrs after the administration of the substrate. Normally, at least 50% of PABA is excreted w/in the 6hrs. this test specificity 96% and sensitivity 75%. Agents contain PABA (suntan lotion, multivitamins) must be avoided before testing. Some drugs (acetaminophen, procainamide, sulfonamides, chloramphenicol, lidocaine, and thiazide diuretics) should be avoided for at least 3 days. - Lactoferrin and trypsin differentiate between pancreatic cancer and chronic pancreatitis. Lactoferrin increase in pancreatic carcinoma: decrease trypsin in chronic pancreatitis. The intubation must be directly in the. Pancreatic duct to avoid misinterpretations since the levels of lactoferrin range from 800-12 000mg/L; pancreatic carcinoma the levels of lactoferrin remain in the normal range 0-400mgL. Decrease concentrations of trypsin are observed in chronic pancreatitis. Tests in acute pancreatitis. - Amylase increase in acute pancreatitis within 24hrs; may be elevated up to 7 days. Amylase is secreted for the purpose of breaking down starches into limited dextrins and maltose. Filtered at the glomerulus and reabsorbed by the kidney tubules. Other conditions that can give rise to increased levels of serum amylase are tubo-ovarian abscess, intestinal obstruction, abdominal trauma, bronchogenic carcinoma, perforated duodenal or gastric ulcer, and intra-abdominal surgery. Amylase is measured in a urine sample that has been collected over 2 hours. Macroamylasemia: pancreatic disease is absent but there is an apparent increase in serum amylase which remains elevated indefinitely. Reference range 16-220 and up to 1200 U/L.
