What work is planned for the release? Consider suitability and possible referral to programmes as well as referrals to partnership organisations. What protective factors are there and what might be put in place? Consider any additional licence conditions that might be appropriate to manage risk of harm, for e.g. victim non-contact, residence, non-association. Depending on the recommendation as to his/her suitability for re-release at the time of the further review, PPCS will make a decision on whether to request updated reports to enable consideration for an executive release to take place. Alternatively PPCS may decide that the offender will be released at his/her SED or that their case will be reviewed by the Parole Board at the point of the statutory annual review date. In the meantime if there are any significant developments which you believe may affect this decision and where re-release is being considered, please get in touch with me. In order to facilitate a release the following information is required by no later than insert date From Probation Annex H and Updated OASYs RMP, including accommodation details ( timescale and availability), clear assessment of suitability for re-release, assessment of motivation to comply, assessment of custodial conduct since return to prison, and any other relevant information. Information regarding victim contact since recall Outcome of mental health assessment (if applicable) Outcome of MAPPA meeting or other risk management meeting/s From Prison - PLEASE COMPLETE ATTACHED DOCUMENT Information regarding overall conduct in prison including outcome of any adjudications and any security concerns Outcome of MDT/VDT's Progress in addressing offending behaviour i.e. completion of offending behaviour groups, contact with CARAT's, counselling etc Outcome of Mental Health/Psychiatric/Psychological assessment (if applicable) This information MUST be emailed directly to me by no later than insert date . Please note that it is your responsibility to ensure that the requested reports are received by the set date. Failure to do so will result in a delay in the offender's case being reviewed .If there is new information that has come to light since the previous review that you believe is relevant to the suitability of re-release you should submit this as soon as possible.
