There have been countless interventions published which show a reduce in errors and a resulting decrease in adverse events. Many of these evidence-based practices are alarmingly simple. For example, one publication reported an 88% in medication administration erros over four weeks by using additional signage on the unit to avoid interuptions during the medication administration process (Tompkins McMahon, 2017). Minimizing harm to patients is the key concept in promoting patient safey. The culture of safety extends beyond the patient. to include a healthy practice environment. The nurse must be empowered to engage in conversations about safety issues and report errors without fear of punishment. As a result of these initiatives, statistics began to show a substantial decrease in the number of adverse events.
